Youth Candidate Election Form

This form is to be filled out by leader who have requested a Unit Visitation from the Order of the Arrow. Please fill the form out entirely being sure not to skip over anything, then hand it to the chairman of the Visitation Team

Adult Candidate Election Form

This form is to be filled out by the Unit Committee Chairperson, Leader, or District or Council Chairperson. Be sure to fill this form out entirely. After completing the form, please mail it to the Lodge Adviser.

Consent Form

This form must be filled out annually by Lodge members

Rimmey Lodge Patch Order Form

This form must be filled out entirely and then submitted at the next Lodge event or at the Council Office.
